Referral Instructions

Patients require a referral from their care provider.

To refer a patient please fax the following information to our office to ensure accurate triage of appointments:

• Referring physician's name and contact information
• Patient's name, demographics, and phone number
• Patient's medications, co-morbidities, and lab results
